Give people hope when they need it most. As a Case Manager in Geelong’s Specialist Homelessness Services, you’ll be a lifeline for those facing or at risk of homelessness — connecting them to vital care, housing, and support. Working with trusted partners like Barwon Health, you’ll help deliver real change and restore dignity, one person at a time.
About the role
We are seeking a dedicated Case Manager for our Barwon homelessness team this role will test your resilience as you walk alongside people facing the harsh realities of homelessness—but for the right person, it offers the rare chance to create lasting change and be part of a team that never gives up on hope.
Reporting to the Support services Team Leader this is a Permanent, full -time role, located in Geelong, Victoria.
Compensation is in accordance with SCHADS Award Level 5.1
You will successfully;
Be the steady voice in the chaos, using you already excellent admin skills conduct client assessments and build tailored case plans that give people a path forward when they need it most.
Champion change, helping clients access safe housing, training opportunities, and vital community networks—because everyone deserves a fresh start.
Walk beside those at their most vulnerable, offering practical life skills, support, and
Shape meaningful futures, creating realistic housing and exit plans that align with best-practice care models and truly make a difference.

You will have
A Social Work qualification or other relevant social tertiary qualifications and/or substantial experience
Experience and resilience in working with complex clients presenting with challenging behaviours
The ability to encourage and cooperate with others to achieve common goals
The ability to acknowledge and respond to a diverse range of experiences, perspectives, values and beliefs
The willingness to complete a national police record check
A Current Victorian Drivers licence
