The Salvation Army (Salvos) is one of the largest providers of homelessness services across the country. Our philosophy is that every person should be afforded dignity and respect and that no one should be without a safe, affordable, and secure home. We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services.
The Role
To provide case management support in a community setting, supporting veterans and their families to secure housing, experience improved social inclusion and achieve housing stability,
This support is achieved through person-centred case management incorporating resilience, trauma-informed and strengths-based practices whilst working effectively with other members of the team to provide a comprehensive service that reflects the vision, mission and values of The Salvation Army.
How You will make an Impact
Establishing relationships with individuals that access the service that encourage self-determination and participation
Providing case management support to individuals in accordance with TSA case management models and standards of practice
Ability to complete thorough assessments, inclusive of needs, risks and vulnerabilities
Ongoing assessment of the individuals’ presenting issues, vulnerabilities and needs from initial intake to exit
Individual case planning in collaboration with the individual including a range of interventions that address the needs or goals of the individual, as well as barriers to achieving those
Implementation of case plans with active individual participation
Assisting individuals to access support services and establish community supports, and where needed provide advocacy for the person with service providers
What You will Bring :
Tertiary qualification in Community Services, Social Work or related field. A degree level qualification is preferred.
Relevant experience in a social service environment including in client assessment and case management
QLD Working with Children Check (position is located at a site that provides overnight accommodation to young people and/or provides support to families in a home setting)
