About the Role – Choices at Home Team Leader
Are you passionate about supporting older people to live independently and stay connected to their community? As the
Choices at Home Team Leader, you’ll lead a dynamic team delivering essential support services through the Commonwealth Home Support Programme (CHSP).
Your key responsibilities will include:
- Planning and coordinating service delivery in line with contracts and budgets
- Implementing processes and procedures to comply with reforms and aged care quality standards
- Promoting Choices at Home to increase awareness and engagement among seniors
- Building partnerships with local NGOs, health professionals, and community groups
- Coordinate team processes for client service agreements, onboarding, assessments, referrals and addressing service gaps
- Recruit and coordinate service volunteers

What You’ll Bring
To be successful in this role, you’ll need:
- Diploma of Community Services (or equivalent)
- Proven experience in team leadership
- Strong background in aged care service delivery
- Excellent organisational and administrative skills
- Current Driver’s Licence and First Aid Certificate
- Ability to obtain a satisfactory criminal records check
