The Opportunity:
The Care Partner collaborates closely with clients and the broader care team to co-design personalised care plans that reflect each client’s goals, preferences, and assessed needs. This role emphasizes promoting functional independence, wellness, and lifestyle choices tailored to individual circumstances. The Care Partner also coordinates with internal and external stakeholders to ensure optimal client outcomes, while working within available resources and budget allocations.
What you will need:
- Certificate IV in Aged Care or Diploma or Degree in Community Services
- Demonstrated experience in home care packaging, case management, assessment, care planning and service coordination
- Understanding of consumer directed, community-based care service delivery
- Understanding of current Aged Care Legislation, Aged Care Principles and Aged Care Standards
- First Aid and CPR Certificate (desirable)
- Experience working with and adhering to budgets
- Ability to liaise with and promote our services to external providers in a proactive and positive manner
- Computer literate (Word, Excel, email, internet)
- Current Driving License
- Demonstrated strong interpersonal skills
- Exceptional customer service skills, with both internal and external customers
- Applicants must have permanent residency or be an Australian Citizen
- Applicants must be willing to undertake a Police Check prior to commencement
